            (If you have been seen by another Provider in the last 6 months and have gotten X-rays taken Please have them email you directly for a copy to have with you for appt or you can sign a release form that we can directly fax)

How to Cancel Your Appointment 

For Appointments Monday through Friday, We advise that you give us at least a 24-hour notice to allow us the time to fill that open spot. There is a late Cancelation fee which is $50 dollars. 

For Appointments that are on our only Saturday a month, we need at least 4 days' notice, again there will be a charge for late cancellations.
